Patient Placement and Transfer Coordinator, RN - Nights

Responsibilities

  • Facilitates patient transfers within the healthcare system, ensuring access to hospitals through the Patient Placement and Transfer Center (PPTC)
  • Manages daily PPTC calls, coordinating with physicians, staff RNs, and departments to meet regulatory standards (COBRA/EMTALA)
  • Ensures workflows adhere to departmental standards, tracks and documents processes, and reports quality issues
  • Matches patient needs with system resources to optimize service value for patients and facilities
  • Provides follow-up communication with internal and external stakeholders to ensure needs are met
  • Uses problem-solving skills to determine optimal actions, prioritizing patient, physician, and hospital satisfaction
  • Demonstrates high customer service standards, maintaining professionalism and helpfulness during interactions

Qualifications & Skills

  • Graduate from an accredited nursing program; BSN required (waivers possible with equivalent experience and approval)
  • Minimum of 3 years healthcare experience, with at least 2 years in acute hospital settings, preferably critical care or house supervisor roles
  • Current RN license in Texas
  • Physical ability to lift, push, and pull patients or objects, with lifting capacity of 10-50 lbs
  • Strong communication, problem-solving, and customer service skills
