About the role
Facilitates patient transfers within the healthcare system, coordinating with physicians, staff, and departments to ensure timely and appropriate placement in compliance with regulations.
Responsibilities
- Manages daily calls to the Patient Placement and Transfer Center (PPTC), ensuring efficient workflows and quality standards are met.
- Matches patient needs with system resources to optimize care delivery and patient outcomes.
- Provides follow-up communications with internal and external stakeholders to ensure service satisfaction.
- Ensures adherence to departmental protocols, documentation, and timelines; reports workflow trends and suggests improvements.
- Uses problem-solving skills to determine best courses of action, prioritizing patient safety and satisfaction.
- Demonstrates exceptional customer service skills, maintaining professionalism and helpfulness during interactions.
Qualifications
- Graduate of an accredited nursing program; BSN required (waivers for ADN with experience may be considered).
- Minimum three years of healthcare experience, including two years in an acute hospital setting, preferably in Adult Critical Care or as a House Supervisor.
- Current RN license to practice in Texas.
- Physical ability to lift, push, and pull patients or objects up to 50 lbs with assistance.
- Strong communication, problem-solving, and customer service skills.
