Patient Placement and Transfer Coordinator, RN - Nights

About the role

The Patient Placement and Transfer Coordinator Registered Nurse (RN) facilitates patient transfers within the healthcare system, ensuring timely access to hospital services through the Patient Placement and Transfer Center (PPTC).

Responsibilities

• Manage daily PPTC calls, coordinate patient placement and transfers with physicians and staff while adhering to regulations (COBRA/EMTALA).
• Ensure workflows meet standards, document appropriately, and track processes.
• Match patient needs with system resources to optimize care, and communicate with internal and external stakeholders.
• Report workflow trends, identify performance improvement opportunities, and apply problem-solving skills to determine best course of action.
• Demonstrate exceptional customer service and professionalism in all interactions.

Qualifications & Skills

• Graduate from an accredited nursing program; BSN required (waivers for ADN with equivalent experience possible).
• Minimum of 3 years healthcare experience, including at least 2 years in acute hospital settings, preferably adult critical care or as a house supervisor.
• Current RN license in the state of practice.
• Ability to lift 10-50 lbs with assistance and perform occasional heavy lifting.
• Strong communication, problem-solving, and customer service skills.
