About the role
Jaan Health is a leading care management company that helps healthcare providers deliver high-quality, personalized care to their patients. The Clinical Care Coordinator (MA) role supports the development of patient-centered, team-based care, and assists primary care physicians (PCPs) and practices in managing their panel of patients.
Responsibilities
- Conduct high-volume patient outreach via phone, text, and digital platforms.
- Document patient interactions and care plan updates in EHRs including Epic and Cerner.
- Support LPNs, RNs and providers with intake coordination, scheduling, and follow-ups.
- Aid in patient education and care plan navigation.
- Escalate clinical issues to LPNs/RNs as appropriate.
- Assist with patient onboarding and enrollment processes.
- Partner with client operations teams to ensure seamless service delivery.
- Identify process improvements to enhance patient and client satisfaction.
- Deliver training sessions on care protocols, systems, and operations to care management teams.
- Adapt to evolving workflows, technology, and program demands.
- Support other operational functions as needed.
- Ensure courteous telephone and digital interactions, accurate and expeditious routing, and referral to appropriate clinical staff when necessary.
- Comply with all applicable federal, state, local, and company policies and procedures.
Requirements
- Certified Medical Assistant required.
- Minimum of 3 years' experience in primary care setting or similar specialty. Population health experience preferred.
- Excellent communication and technology skills.
- Prior use of electronic health records (Epic) and other health care information systems desirable.
- Significant experience within a primary care setting (chronic care preferred, hospital, medical groups).
- Proven problem-solver, comfortable with high-volume outreach, and able to multi-task.
Qualifications
- High school diploma or equivalent.
- Current certification as a Medical Assistant.
- Experience with electronic health records (Epic) and other health care information systems.
- Experience in a primary care setting, preferably with chronic care.
- Ability to handle high-volume tasks and multi-task effectively.
- Strong problem-solving skills.
