Job Summary

The Clinical Care Coordinator plays a dual role within the healthcare practice, seamlessly blending clinical support with care coordination. This position ensures patients receive compassionate, holistic, and well-organized medical care by assisting providers during clinical procedures while also coordinating health and social services that support overall well-being.

Qualifications

  • Certification as a Medical Assistant (CMA or RMA) required.
  • Associates Degree.
  • Experience in patient care coordination, case management, or community health.
  • Strong understanding of infection control and clinical protocols.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work collaboratively across clinical and administrative teams.
  • NHA Certification (e.g., CCMA/CMA - Certified Clinical Medical Assistant) preferred.
  • Computer savvy - Word, Excel, PowerPoint, Adobe.
  • Bilingual Spanish strongly preferred.

Pay Rate

$22.00-$25.00

Location

Required to be in Bronx-based office location 5 days a week.

About the Role

Guided by a Population Health model, our team of 500+ providers deliver care in-person, at home, or via telehealth, ensuring patients get the support they need when and where they need it.

Responsibilities

  • Perform patient triage to assess and prioritize medical needs.
  • Absorb and assist healthcare providers with in-house procedures such as EKGs and PFTs.
  • Ensure examination areas are sterile, organized, and properly equipped.
  • Sterilize and autoclave medical instruments; uphold infection control protocols.
  • Track and manage inventory of medical supplies; ensure adequate stock.
  • Maintain accurate documentation for equipment usage and procedures.
  • Build and maintain positive relationships with patients, caregivers, and care teams.
  • Conduct in-person assessments to evaluate medical and social needs.
  • Develop and implement individualized care plans in collaboration with providers.
  • Promote timely access to care and increase preventive care utilization.
  • Schedule medical appointments and coordinate transportation.
  • Educate patients on community resources and refer as needed.
  • Act as a point of contact, advocate, and resource for patients and families.
  • Maintain detailed, up-to-date patient records and coordination logs.
