Clinical Care Coordinator
About the role
The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key roles include coordination of services for panel patients, focusing on those who face particular difficulty with navigating the healthcare system, management of specialty referrals, and supporting patients in self-care and identifying relevant community resources. The Clinical Care Coordinator collaborates with and outreaches to outside service providers to provide seamless transitions of care, and provides patient education while assisting with access to applicable resources.
Responsibilities
- Coordinate services for panel patients, focusing on those facing navigation difficulties.
- Manage specialty referrals and support patients in self-care.
- Identify and connect patients with relevant community resources.
- Collaborate with and outreach to outside service providers to ensure seamless transitions of care.
- Provide patient education and assist with accessing applicable resources.
Requirements
Preferred qualifications include an Associate's Degree or equivalent education/experience, two years of healthcare experience, experience in an outpatient setting, and experience in case management.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $23.81 - $36.44.
