Responsibilities
- Coordinates care activities among participants, providers, support teams, and community resources.
- Facilitates communication between behavioral health providers, community partners, and internal program teams.
- Promotes integrated, person-centered service delivery across multiple service settings.
- Supports participants in accessing appropriate behavioral health, medical, social, and community-based services.
- Develops and maintains collaborative partnerships with external providers and community agencies.
- Conducts routine reviews of participant services to ensure alignment with treatment goals.
- Assists with identifying barriers to care and implementing solutions to improve participant outcomes.
- Maintains current knowledge of organizational programs, services, and behavioral health initiatives.
- Ensures familiarity with applicable regulatory, licensing, accreditation, and funding requirements.
- Makes ongoing monitoring of participant needs and service utilization.
- Participates in quality improvement initiatives and program evaluation activities.
- Completes provider outreach and coordination activities through routine visits and stakeholder engagement.
- Supports implementation of best practices and evidence-based approaches to care coordination.
- Develops comprehensive treatment plans for individuals receiving services across multiple settings.
- Reviews treatment plans to ensure regulatory compliance, appropriateness, and quality.
- Facilitates routine interdisciplinary treatment team meetings and collaborative planning sessions.
- Conducts systematic reviews of participant records and treatment progress.
- Evaluates service effectiveness and identifies opportunities for improvement.
- Ensures participant records contain appropriate documentation and linkage to available resources.
- Assists with care plan updates and treatment recommendations based on identified needs.
- Supports quality assurance activities related to participant care and treatment outcomes.
- Provides direct supervision to Care Coordinators and Resource Coordinators.
- Conducts ongoing coaching, mentoring, and performance support.
- Promotes professional development and continuous learning among team members.
- Assists staff with problem solving, case consultation, and resource identification.
- Provides guidance related to organizational policies, procedures, and best practices.
- Facilitates regular staff meetings and case review discussions.
- Supports onboarding and training activities as assigned.
Qualifications
- High School Diploma or GED.
- Minimum three (3) years of relevant behavioral health, human services, case management, care coordination, or related experience.
- Preferred Bachelor’s Degree in Human Services, Social Work, Psychology, Behavioral Health, Counseling, Sociology, or a related field.
- Minimum two (2) years of experience working with: Behavioral Health populations, Individuals with Mental Health diagnoses, Intellectual and Developmental Disabilities (IDD), Substance Use Disorder (SUD) treatment populations.
- Strong verbal and written communication skills.
- Strong organization and documentation skills.
- Ability to coordinate multiple services and providers effectively.
- Strong analytical and critical-thinking abilities.
- Ability to lead and supervise support staff.
- Knowledge of behavioral health systems and treatment planning.
- Ability to maintain confidentiality and professionalism.
- Ability to manage competing priorities in a fast-paced environment.
- Strong interpersonal and relationship-building skills.
- Proficiency with Microsoft Office applications and electronic documentation systems.
