Summary

The Clinical Coordinator reports to the Director of Behavioral Health. Key responsibilities include supporting patient care, coordinating care with medical providers, assessing patients, facilitating treatment plans, managing referrals, and ensuring compliance with standards and regulations.

Essential Duties and Responsibilities

  • Support the mental and physical health care of patients on behavioral health units.
  • Carefully screen and assess patients for common mental health and substance abuse disorders.
  • Facilitate patient engagement and follow-up care, providing patient education about treatment options.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects.
  • Provide or facilitate referrals to evidence-based psychosocial treatments and other services as clinically indicated.
  • Track patients' follow-up and clinical outcomes, documenting in-person and telephone encounters and treatment recommendations in EHR and other systems.
  • Facilitate treatment plan changes for patients not improving as expected, consulting with medical providers and psychiatric consultants.
  • Develop and complete relapse prevention self-management plans for patients achieving treatment goals.
  • Perform other duties as assigned.

Compliance Responsibilities

  • Understand and adhere to Loretto Hospital’s compliance standards.
  • Stay informed of relevant federal, state, and hospital regulations, laws, and policies.
  • Ensure staff are trained and evaluated on compliance policies and procedures specific to their roles.

Requirements

  • Demonstrated ability to collaborate and communicate effectively in a team setting.
  • Effective communication and relationship-building skills with patients and care team members.
  • Experience with screening and differential diagnosis of common mental health and substance abuse disorders.
  • Knowledge of evidence-based psychosocial treatments and brief behavioral interventions.
  • Basic understanding of psychopharmacology for common mental health disorders.
  • Graduate of an accredited school of nursing; B.S.N. preferred.
  • Current RN license for the State of IL.
  • BLS license and CPI within 60 days of hire.
  • Two years’ experience in mental health or a combination of mental health and chemical dependency preferred.

Physical Demands

While performing the duties of this job, the employee is frequently required to sit, stand, and walk. Occasionally stoop, kneel, crouch, or crawl. The employee may be expected to lift, pull, push, or move patients and/or equipment. Fine motor skills are required for handling, feeling, or reaching tasks. Normal or corrected vision, hearing, and the ability to speak are necessary. Specific vision abilities include close vision, sharp eye focus, and color differentiation. There will be periods of stressful intense concentration with more than average pressure used in a variety of problem-solving situations.
