Core Responsibilities & Expectations for the Role
- Oversee daily clinic operations within the PACE clinic, ensuring high-quality, participant-centered care for both chronic and acute needs.
- Assign duties to clinic staff and provide guidance during shifts to ensure tasks are completed timely and accurately including oversight of EMR to ensure appropriate scheduling and documentation.
- Provide leadership oversight and performance management including feedback, coaching, and mentoring to clinic team members ensuring role clarity to perform functions at the top of their license.
- Support onboarding, training, and ongoing education for RNs, LVNs, and MAs
- Manage clinic schedules to maintain coverage
- Provide support with quality initiatives, audit readiness and monitoring CAPs in partnership with Clinic Services Manager and the Medical Director
- Maintain supply levels and handle day-to-day operational needs including vendor management, supply ordering, and facility/equipment issues.
- Ensure adherence to quality and compliance standards of Habitat Health and other regulatory agencies.
- Provide after-hours coverage as needed.
- Provide clinical care as needed such as nurse triage, emergency response, vaccine and medication administration, wound care, home care, and patient education
Required Qualifications
- Must hold a current, unrestricted California RN
- Graduate of an accredited school of nursing with two (2) – five (5) years of experience leading nursing teams, delegating tasks, and managing clinic operations.
- Strong communications skills with the ability to build trust and rapport within a team.
- Demonstrated ability to manage schedules and prioritize work to keep operations running smoothly.
- Demonstrated ability to make routine clinic decisions, solve problems in real time, and escalate issues as necessary.
- Experience with caring for a complex geriatric population with chronic conditions
- Strong knowledge of clinical quality and compliance standards

Essential Functions & Physical Requirements
- Work in healthcare, administrative, community, or remote environments, depending on role needs.
- Communicate effectively with participants, caregivers, team members, and other stakeholders in person, by phone, and through electronic systems.
- Remain stationary and/or move about for extended periods, consistent with job duties.
- Operate standard office equipment and/or clinical tools (e.g., computers, phones, medical devices, documentation systems).
- Travel between work sites or community locations as needed.
- Follow safety procedures, infection control protocols, and use personal protective equipment (PPE) when required.
- Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities.
- Lift, carry, push, or pull supplies, equipment, or materials typically up to 25–50 pounds, with assistance or mechanical aids, depending on job duties and role responsibilities.
Compensation
- The anticipated base salary range for this position is $120,000 - $137,000 + a 10% bonus.
- The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
