Responsibilities

  • Career Caregivers: Develop an amazing team of compassionate and dedicated healthcare providers.
  • Client Care Coordination: Coordinate and oversee the delivery of personal care services in alignment with client plans of care.
  • Record Management: Maintain accurate and up-to-date records for client services, tasks, schedules, and caregiver assignments.
  • Schedule Management: Build, maintain, and update caregiver schedules to ensure appropriate coverage and continuity of care.
  • Documentation: Review and maintain contract-specific documentation and ensure authorizations align with scheduled services.
  • Client Education: Educate clients on Elara Caring policies and applicable state and federal regulations.
  • Service Monitoring: Monitor daily service delivery to ensure adequate caregiver availability and timely staffing of open shifts.
  • Guidance and Support: Provide guidance, support, coaching, and direction to caregivers to promote quality care and performance improvement.
  • Compliance: Ensure caregiver compliance with training, certifications, in-services, health requirements, and regulatory standards.
  • Orientations: Conduct caregiver orientations as required and partner with Human Resources on disciplinary actions when necessary.
  • Reporting: Review reports to ensure accuracy in billing, payroll, utilization, and service delivery.
  • Timekeeping: Collaborate with payroll and timekeeping teams to resolve timesheet discrepancies.
  • Incident Reporting: Review utilization reports, identify trends, and address inconsistencies on a daily basis.
  • Documentation: Review and submit incident reports and workers’ compensation documentation as required.
  • Communication: Communicate professionally and effectively with caregivers, clients, case managers, and internal leadership.
  • Performance Concerns: Consult with leadership regarding employee performance concerns, client issues, or operational risks.
  • Confidentiality: Maintain confidentiality and compliance with HIPAA and all regulatory, licensure, and contract requirements.
  • Branch Growth: Participate in branch growth initiatives, operational planning, and professional or community engagement activities.
  • Professionalism: Represent Elara Caring in a positive, professional manner at all times.

Requirements

  • High School Diploma or GED required.
  • Associate’s or Bachelor’s degree preferred.
  • Two (2) years of experience in a fast-paced office or care coordination environment preferred.
  • Experience working with elderly or disabled populations preferred.
  • Working knowledge of Medicare/Medicaid and home care benefits preferred.
  • Strong computer proficiency and documentation skills.
  • Excellent organizational and multitasking abilities.
  • Professional verbal and written communication skills.
  • Team-oriented with leadership and motivational abilities.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of the healthcare industry preferred.
