About the role
Mae is a venture-backed digital health solution dedicated to improving maternal, baby, and family health. As a Care Coordinator, you will support Medicaid payer pilots and member-level engagement to address disparities in Black maternal health outcomes.
Responsibilities
- Conduct needs assessments and screeners, including psycho-social, physical, medical, behavioral, environmental, and financial parameters
- Utilize influencing and motivational skills to ensure maximum member engagement
- Serve as the liaison between the member, Mae team, and partner care management teams, ensuring all documentation is completed
- Manage care plans and referrals with payer care management teams and serve as the point of contact to ensure services are rendered appropriately
- Manage communication and relationships with members through phone, email, and presentations
- Promote the appropriate use of clinical and social structural benefits among members
- Identify and escalate quality of care issues through established channels
- Facilitate a team approach and educate the team on a member-focused approach to care
- Identify high-risk factors and service needs that may impact member outcomes and care plan components
Requirements
- Highly organized, able to manage time effectively, and comfortable working across multiple workstreams concurrently
- Comfortable managing outreach responsibilities across different clients
- 3-5 years of experience in social work, nursing, or a healthcare-related field
- Knowledge of referral coordination to community and private/public resources
- Relationship management experience
- Demonstrates efficiency and consistency in meeting daily outreach quotas
- Is tech-savvy and confident using computer systems to manage and document outreach
- Strong written and verbal communication and presentation skills
- Can manage time effectively and work both independently and collaboratively
- Has strong communication skills and is comfortable speaking with clients over the phone
- Birthwork/Doula experience a plus
- Bilingual (English & Spanish) required
Qualifications
- 3-5 years of experience in social work, nursing, or a healthcare-related field

Benefits
Flexible schedule, 40 hours/week, Monday-Friday. Hourly rate $32 - $38, depending on experience and qualifications.
