Care Coordinator (Bilingual English / Spanish)

Your Role & Impact

• Patient Care & Enhancing Care Flow
• Use assessment results and provider input to refer patients to community resources
• Help patients to connect to primary care, specialty, and ancillary services
• Encourage patients to become actively engaged in their own health
• Communicative Support & Communication
• Communicate with care team, review patient charts, identify gaps in care, and participate in establishing the care plan
• Maintain timely and consistent communication with patients regarding their health care and resource needs
• Engage in community outreach with neighborhood schools, churches, and other business partners

Qualifications

• Associates Degree or equivalent work experience, required
• Three or more years of clinical experience, required
• Experience as a Medical Assistant, Case Manager, or Health Educator, preferred
• Previous experience with Electronic Health Records, preferred
• Knowledge of Microsoft Office (Excel, Word), preferred
• Bilingual English/Spanish, required
