About the role

The Client Care Coordinator (CCC) plays a vital role in delivering exceptional service to seniors and their families throughout the Lubbock area while supporting the continued growth and success of the office. In this role, you will collaborate with a dedicated team of caregivers and healthcare professionals to provide personalized, one-on-one care that enhances our clients' quality of life and fulfills our mission to Elevate the Human Spirit.

The CCC manages the client journey from initial referral through ongoing care coordination. This includes building and maintaining relationships with referral partners such as assisted living and independent living communities, hospice agencies, geriatric care managers, medical specialists, and other senior-focused organizations. The coordinator also serves as a trusted resource for prospective clients by responding to inquiries, educating families about available services, and scheduling in-home consultations. Once services begin, the CCC oversees the continuity of care by coordinating communication and resources among clients, families, community providers, insurance companies, case managers, and other third-party payers. Working closely with HR & Scheduler, the CCC helps ensure caregiver skills and qualifications are aligned with client needs, creating a seamless and exceptional care experience.

Responsibilities

  • Develop and maintain referral relationships within the senior care and healthcare community
  • Respond to prospective client inquiries and educate families about services and care options
  • Schedule and conduct client intake activities and in-home consultations
  • Coordinate ongoing care and serve as a primary point of contact for clients and families
  • Collaborate with community providers, insurance companies, case managers, and other third-party payers
  • Partner with HR/Scheduler to match qualified caregivers with client needs
  • Support office growth through outstanding client service and relationship management
  • Ensure every client receives a personalized, high-quality care experience

Requirements

  • Minimum of high school diploma or GED
  • Minimum of 2 years of related experience
  • Strong computer literacy, including proficiency in Microsoft 365 (Word, Excel, PowerPoint), DocuSign, and scheduling/CRM platforms (experience with VIV or similar systems is a plus)
  • Ability to demonstrate leadership skills and outstanding people management skills
  • Attention to detail
  • Exceptional customer service skills and written / verbal communication
  • A reliable vehicle and valid driver’s license with the ability to pass a background check (including motor vehicle records check)
  • Ability to manage personal stress and consistently present a positive, professional image
  • Participates in on-call rotation as assigned
  • Other duties as assigned

Physical Demands
