Avera Granite Falls Health Care Center – Regular, primarily evening shift with rotating weekends and holidays.
Pay Range
$19.75 - $26.00 per hour, dependent upon experience. Position may be eligible for a sign-on bonus up to $500.
Schedule
- Evening shift: 2 p.m. – 10:30 p.m.
- 32 hours per week
- Every other weekend and holiday rotation
About the Role
- Accountable for providing direct, personal care and services to residents in the skilled nursing facility.
- Meet residents’ physical, emotional, social, and spiritual needs daily to maintain and improve their highest level of independent function.
- Work as part of a multidisciplinary team focused on compassionate, high-quality care.
Responsibilities
- Assist residents with activities of daily living (bathing, dressing, oral hygiene, toileting, feeding, ambulation, and transfers).
- Perform routine and non-routine duties as directed by nursing staff, including bed making, answering call lights, and maintaining a clean environment.
- Measure and document vital signs (blood pressure, pulse, temperature, respiration, weight).
- Assist residents with specific medical care activities and restorative exercises as outlined in the care plan.
- Collect specimens and provide immediate feedback to nursing staff on resident status, pain management, or other concerns.
- Restock supplies, perform cleaning procedures, and maintain a safe work environment.
- Provide emotional, educational, and spiritual support to residents and families.
- Uphold resident rights, confidentiality, and privacy; report any allegations of abuse or misappropriation.
- Follow established policies for fire, tornado, evacuation, disaster plans, universal precautions, infection control, and hazardous materials.
Requirements
- Active Minnesota CNA certification or enrollment in a MN CNA class ending within 30 days of applying.
- Ability to work specified hours, including evenings, weekends, and holidays.
- Visual acuity and communication skills sufficient to perform position duties and interact effectively with residents, families, and staff.
- Ability to perform essential job functions with or without reasonable accommodations.
Preferred Qualifications
- High school diploma or GED equivalent.
- Less than 1 year of related experience or training.
