Responsibilities
- Provided comprehensive care and coordination of services for clients and families enrolled in the program
- Implemented treatment plan and tracked progress of clients toward their goals
- Worked collaboratively with Healthy Transitions team to promote, improve, and sustain the overall wellness and physical health of the target population
- Coordinated specialty care evidence-based treatment model
- Provided case management interventions such as advocacy, resource linkage, skills building, support, and interventions to clients in their home and community
- Aided clients in accessing eligible benefits or services, developing social and natural support networks, and applying skills in real-life environments
- Functioned as a liaison for clients with Healthy Transitions team, community agencies, schools, employment sites, and other providers
- Supported clients as they worked towards their employment and educational goals
- Attended coordinated care meetings and reported issues raised to the HT Team Leader
- Used creative approaches and Motivational Interviewing to engage clients who may be ambivalent about participating in treatment
- Provided individual and group interventions as identified in the treatment plan, in the home, at a Center facility, and in the community
- Assisted clients in managing prescribed medications, supporting them in attending appointments, maintaining medication supply, monitoring side effects, and communicating with prescribers
- Copied care with other providers as prescribed in the treatment plan
- Maintained ongoing and collaborative relationships with clients, team members, and community providers and partners
- Maintained knowledge of available providers and resources in the community to address specialized needs
- Participated in case consultation, collaborative care meetings, and integrated treatment planning
- Attended agency and Healthy Transitions staff meetings as scheduled
- Attended weekly supervision as scheduled
- Maintained detailed, accurate, and timely documentation in medical records
Qualifications
- Associate’s degree, required. Bachelor’s degree, preferred
- At least 2 years' experience working with adolescents or young adults in a mental health or school setting
- Knowledge of adolescent development and family systems issues
- Knowledge of community resources for clients and families with behavioral health disorders
- Advocacy and teaching skills
- Valid driver's license and use of car during normal working hours or other means of transportation in order to perform the major functions of the job
- Automobile liability insurance coverage for vehicle being used in accordance with Center requirements
- CPR training obtained within 90 days of employment and maintained annually
- Ability to work collaboratively as a team to coordinate care for youth and young adults with intensive needs
