Responsibilities

• Provide community outreach services to adults with chronic behavioral health issues, including substance use disorders, schizophrenia, dual diagnosis, schizoaffective disorder, and physical issues.
• Work collaboratively with a multidisciplinary team consisting of master-level clinicians, nursing staff, peer specialists, and others involved in client care.
• Ensure the welfare of a large caseload of clients by advocating for their needs and recommending appropriate resources.
• Make assessments, address client needs, and recommend treatment options.
• Serve as the primary liaison between clients and their families/guardians.
• Collaborate closely with the team of providers, providing transportation, administering medication, managing client finances, and documenting client health records.

Requirements

• A current valid driver's license in good standing, along with driving experience and an excellent driving record, for client transportation.
• Reimbursement for gas and mileage provided.
• CPR/First Aid and MAP certifications are required.

Qualifications

• An HS Diploma or BA degree in Psychology or a related field is preferred.
• One or more years of experience in the Mental Health field.
• Working knowledge of community-based treatment, funding sources, and treatment regulations is strongly preferred.

• Full-time, outreach position with a combination of 9a-5p (3 days) and 11a-7p shifts weekly.
