About the role

The Case Manager at the West Scattered Site is responsible for providing comprehensive support to individuals experiencing homelessness. This includes case management services, outreach, and coordination with various community resources.

Responsibilities

• Provide individualized case management services to clients.
• Coordinate access to housing, healthcare, and social services.
• Conduct outreach to locate and engage homeless individuals.
• Assist clients in accessing emergency shelter, transitional housing, and permanent supportive housing.
• Document client progress and maintain accurate records.
• Maintain positive relationships with clients, service providers, and community partners.

Requirements

• Bachelor’s degree in Social Work, Public Health, or related field.
• At least 2 years of relevant experience in case management or social services.
• Valid driver’s license and reliable transportation.
• Ability to work flexible hours including evenings and weekends.
• Proficiency in Microsoft Office Suite.

Schedule

The Case Manager works Monday through Friday, with some evening and weekend shifts required.
