About the role
The Case Manager will provide direct services to individuals experiencing homelessness, focusing on case management, outreach, and support.
Responsibilities
- Provide individualized case management services to clients
- Conduct outreach to locate and engage homeless individuals
- Assist clients with accessing necessary resources such as housing, healthcare, and social services
- Maintain accurate and up-to-date records of client interactions and progress
- Participate in team meetings and collaborate with other staff members
- Ensure compliance with relevant policies and procedures
Requirements
- Bachelor’s degree in Social Work, Public Health, or related field
- At least 2 years of experience in case management or related field
- Valid driver’s license and access to reliable transportation
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite

- Experience working with individuals experiencing homelessness
- Knowledge of local resources and services for homeless individuals
- Ability to maintain confidentiality and handle sensitive information
- Strong organizational and time management skills

Pay
Salary range: $30,000 - $40,000 annually
Schedule
Full-time position, Monday through Friday, 8:00 AM - 5:00 PM
