About the role
The Case Manager at the West Scattered Site is responsible for providing comprehensive support services to individuals experiencing homelessness. This includes case management, advocacy, and coordination of resources.Responsibilities
- Assess and document individual needs through intake assessments.
- Develop and implement personalized service plans.
- Coordinate access to housing, healthcare, and social services.
- Advocate for clients' rights and interests.
- Maintain accurate records and documentation.
- Participate in team meetings and trainings.
Requirements
- Bachelor’s degree in Social Work, Public Health, or related field.
- Minimum of 2 years of relevant experience in case management or social services.
- Valid driver’s license and reliable transportation.
- Proficiency in Microsoft Office Suite.
- Excellent communication and interpersonal skills.
