Responsibilities
- Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, exit summaries and follow-up.
- Manage a case load.
- Maintain familiarity with program resources available on- and off-site.
- Cookordination client services with other social service, mental health, medical, employment, educational, childcare, and other providers.
- Conduct regular case conferences with clients and service providers as required.
- Complete program reports monthly, or more frequently as required.
- Report to, and meet with, supervisor monthly.
- Conduct home visits and other fieldwork, as required.
- Perform additional duties as assigned.
Program Specific Responsibilities
- Conduct intake and assessment to determine client eligibility based on risk of eviction and vulnerability and understand events that precipitated the crisis leading to eviction proceedings.
- Develop a customized service plan with clients, outlining action steps and responsibilities.
- Accompany seniors to court proceedings as required and support them through the court process providing information about how the system works and what they can expect.
- Aid the client with obtaining necessary documentation for court and for accessing rental income subsidies or other forms of additional income.
- This includes increasing the number of selected benefits, renewal applications, or the recertification process associated with the continuation of key benefits.
- Maintain up to date case files and database entries, as well as regular contact with clients via office visits, telephone contact and home visits.
- Cookordination client referrals, and maintain close links with the Emergency Rent Coalition, which includes all the charitable organizations that have arrears funds.
- Provide budget counseling to ensure effective money management skills.
- Refer clients to alternative suitable housing and provide support to manage the transition and follow up support where required.
- Once eviction is prevented, and all available subsidies are obtained, establish continued visits for up to six months to ensure identified housing risks are reduced.
- Other duties as assigned.
Qualifications
- Highschool Diploma, bachelor’s degree preferred.
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile, and copier equipment.
- One year’s experience is preferred.
Physical Requirements
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time, or climb five flights of stairs, if required to conduct home visits and/or fieldwork.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
