About the role
The Case Manager will work with individuals experiencing homelessness to provide support services and assist them in accessing resources.Responsibilities
- Conduct assessments and develop individualized service plans
- Coordinate access to housing, healthcare, and social services
- Maintain accurate records and documentation
- Provide crisis intervention and counseling
- Engage with community partners to enhance service delivery
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- At least 2 years of relevant experience in case management or social services
- Licenses/certifications: CPR, PHR, SPHR
Qualifications
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office Suite
- Experience with data entry and record keeping
Skills
- Excellent organizational and time management skills
- Knowledge of local homeless services and resources
- Ability to handle sensitive and confidential information
