Case Manager - St. Lucy's
Job Duties
- Deliver social services, intake, assessments, admissions, discharge, develop & implement Service Plans, homeless prevention services and related documentation and progress reports
- Maintain compliance with HUD, County and DCA essential services standards and related policy and procedures
- Maintain referrals and communication with County Coordinated Assessment Agency
- In partnership with the family, develop a Housing & Service Plan and provide assistance and linkages in obtaining needed services and achieving clients' goals
Job Requirements
- Bachelor's Degree required (Bachelor’s degree in social work or related field preferred)
- 2-3 years experience working with the homeless population or related experience required
