Responsibilities
- Coordinates and collaborates with physicians, providers, multidisciplinary team, and other healthcare professionals regarding patient’s goals, plan of care, and progress.
- Develops, implements, interprets, and maintains work standards and procedures compliant with ARH’s policies and governmental regulations and various regulatory agency requirements.
- Maintains up-to-date knowledge in the field to recommend new services, products, and equipment.
- Assesses, develops, implements, and monitors comprehensive plans of care through an interagency multidisciplinary team process in collaboration with the patient and family in internal and external settings.
- Revise and adjust the plan of care daily based on ongoing assessments of the patient’s condition.
- Assumes responsibility and accountability for the care plan and its effectiveness and patient outcomes.
- Assesses the appropriateness of the level of care, diagnostic testing, clinical procedures, quality, and clinical risk issues, and ensures medical record completeness.
- Develops, implements, monitors, and evaluates clinical pathways and clinical pathway variances.
- Maintains and monitors patients’ progression through clinical pathways.
- Communicates continuously with patients, families, medical staff, caregivers, and third-party payors as necessary.
- Aids patients and families with the educational process before admission, during hospital stay, and after discharge, as required.
- Develops and maintains a positive work climate and supports the overall team effort of the hospital.
- Assists in collecting and analyzing outcome data.
Qualifications
- Education: Associate's Degree in Nursing preferred, but experience can be substituted.
- Work Experience: 4-6 years preferred, but education can be substituted.
- Knowledge, Skills, and Abilities: Advanced problem-solving and decision-making skills; strong multi-tasking and flexibility; excellent customer and community relations; advanced communication and leadership skills; advanced execution and delivery skills; ability to consider costs and benefits; mastery of care transitions and resources; digital literacy; ethical handling of sensitive information.
- Licenses and Certifications: Registered Nurse preferred, Licensed Practical Nurse (LPN) required.
