About the role
This role involves collaborating with healthcare team members to coordinate appropriate, cost-effective services that align with members' care plans, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, problem-solving, financial counseling, and access community resources.
- Manage and coordinate caseloads, develop and monitor care plans to promote optimal functioning.
- Advocate for resources, ensure timely implementation, and document activities in compliance with standards.
- Analyze outcomes to improve program quality and cost efficiency.
- Assist with application processes for public programs and refer to community agencies as needed.
- Provide counseling, education, and support to members, families, caregivers, and healthcare teams, including end-of-life discussions.
- Schedule family meetings, complete guardianship paperwork, and provide technology support for virtual court attendance.
- In inpatient settings, complete assessment instruments as required.
Qualifications
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensing from multiple states, including required credentials at hire and additional credentials post-hire.
- Licenses in DC, Maryland, and Virginia within specified timeframes.
Skills
- Experience with Windows-based software.
- Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
