Case Manager - PHP

Responsibilities

  • Provides community-based mental health case management.
  • Assists in assessing client progress, planning treatment interventions and documenting therapeutic activities.
  • Engages, supports, and counsels clients in partial care groups and individual therapeutic interventions.
  • Participates in treatment team planning and review, maintaining continuity of care between community educators and treatment providers as well as the program.

Qualifications

  • Bachelor’s in social work, psychology, rehabilitation or a related field.
  • Associate’s degree candidates currently enrolled in Bachelors degree programs and with additional experience will be considered on a case by case basis.
  • A minimum of two years relevant experience in a mental health setting.
  • Associates degree candidates must have at least four years of relevant mental health experience.
  • Individuals working with Adolescents must have at least one year of experience working with youth in a mental health setting.
  • Candidates for the position within Residential Services that have less than 2 years experience will be considered on a case by case basis.
  • Basic understanding of mental health services and role of case managers in mental health services.
  • Behavioral Violence Prevention (BVP) within 6 months of hire and maintain current.
  • Valid New Jersey Drivers License.

Pay

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $20.50 to $22.93. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
