Case Manager

About the role

CSAP Program (Crisis Stabilization and Assessment) Hammonton is a short-term intensive treatment home for adolescents with neurodevelopment/psychiatric/medical needs. The Transition Specialist will join a multidisciplinary team including a Psychiatrist, Board Certified Behavior Analyst, and Residential Counselors who provide a range of therapeutic services.

Responsibilities

• To assist youth and their families in identifying, accessing, and receiving appropriate mental health and other services in a timely, efficient, and effective manner.
• Participate in the assessment, evaluation, and treatment of clients and families under the supervision of a licensed social worker.
• Compile psychosocial history.
• Develop individual service plans in collaboration with collaborative agencies, staff, guardians, and clients.
• Provide group therapy sessions weekly.
• Engage in client-centered advocacy, assessing, and ensuring quality care is provided.
• Make referrals and linkages to ancillary and specialized treatment services and interventions.
• Represent clients on the agency’s behalf in screening, placement, court appearances, and other issues.

Requirements

• A Bachelor's degree with 3 or more years of relevant experience working with individuals with Intellectual and Developmental Disabilities (I/DD).
• An unlicensed Master's degree with 1 year of experience.

• Ability to work independently and as part of a team.
• Knowledge of relevant laws and regulations.
