Case Manager
About the role
The Salvation Army is seeking a full-time case manager for the Joseph McFee Center (JMC), a recovery-focused transitional housing program in San Francisco. The JMC aims to help participants build on their progress in early recovery and achieve a healthy, independent, and prosperous life.
Responsibilities
- Manage an assigned caseload of up to 25 program participants
- Conduct interviews and administer assessments to understand client needs, situations, and potential risks
- Gather relevant information about the client’s background, family dynamics, living conditions, or any other relevant factors
- Create an individualized action plan with specific goals and objectives based on the assessment
- Collaborate with the client to establish and maintain relationships with partner organizations and referring agencies
- Maintain and reassess client progress to ensure the goal plan remains relevant and effective
- Educate and empower clients by equipping them with the knowledge and skills needed to improve their situation and become more self-sufficient
- Respond to and manage crisis situations clients may encounter
- Maintain appropriate, professional, and ethical boundaries with staff and clients
- Be punctual, flexible, and reliable
- Maintain accurate client records on The Salvation Army Social Services Database
- Attend all required staff, Corps, and Divisional meetings and trainings
- Facilitate small group instruction as indicated on the program schedule
- Attend community-based meetings as needed
- Identify appropriate resources, programs, and services that can benefit the client and make referrals to these services
- Collaborate with various service providers to ensure the holistic needs of clients are met
- Assist in the planning, preparation, and execution of special functions such as graduations, holiday celebrations, group activities, or special classes
Qualifications
- High school diploma. College course work in social services or psychology preferred
- Strong verbal and written communication skills
- The ability to express empathy and foster a collaborative alliance with clients
- Familiarity with San Francisco social services, government resources, community resources, healthcare services, mental health support, and housing services
- Excellent organizational skills and ability to work with little direct supervision
- Culturally competent and able to work with a highly diverse population
- Excellent time management skills
- Valid CA Driver License and completion of organization’s driver safety training within introductory period (desired)
- Experience and ability working successfully with issues of substance abuse, mental illness, and other potential barriers to self-sufficiency (desired)
- Bilingual in Spanish preferred (desired)
- Knowledge of co-occurring disorders and trauma-informed environment (desired)
- Strong electronic communication skills (e.g., Outlook) (desired)
- Proficient in Microsoft Word & Excel (desired)
- Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead (physical requirement)
- Ability to lift 25 lbs (physical requirement)
- Ability to operate telephone (physical requirement)
- Ability to type 35 words per minute (physical requirement)
Compensation
$28 - 31 (hourly)
