Bonita House, Inc. (BHI) is a nonprofit mental health agency with multiple sites in Alameda County and an operating budget of $14.5 million. Founded in 1971, BHI provides social rehabilitation including a full range of Medi-Cal services to adults with co-occurring serious mental health and substance use disorders. Responsibilities
- Works collaboratively on a multi-disciplinary team to coordinate treatment and supportive services for up to 30 adults with co-existing psychiatric and substance use conditions in all stages of treatment and recovery.
- Provides intensive outpatient case management, rehabilitative therapy and support to adults with mental health and/or substance use symptoms.
- Helps organize and lead social and recreational activities to help clients structure their time, increase social experience, and provide opportunities to practice social skills.
- Completes individual therapy, biopsychosocial assessments including ANSAs and sound clinical documentation that supports client treatment plans.
- Participates in clinical supervision with the agency QI Clinical Supervisor in order to ensure appropriate treatment and sign off on assessments as well as gain hours towards licensure if applicable.
- Assists clients in developing an individualized transition plan to facilitate exit from behavioral health system or transition to lower level of mental health services.
- Performs risk assessments, consulting with supervisor and team clinicians as needed, and develops crisis management plans targeting problematic behaviors and delivers effective treatment interventions to reduce the risk of harm to self and/or others.
- Conducts outreach to engage the client in services and treatment, and when appropriate, family members, to support the client.
- Provides psycho-education to clients and when appropriate, family members, related to health, well-being, and lifestyle, including symptom management, medication, diet, sleep hygiene, trauma-informed care, and relapse prevention; assists clients in the creation of personal WRAP plans, and/or Advance Directive.
- Provides ongoing review of a client’s mental status, response to treatment, and progress in recovery. Initiates modifications to the treatment plan to ensure appropriate interventions are provided in response to changes in the client’s mental status, behavior or stage of recovery.
- Assists clients in developing an individualized transition plan to facilitate exit from behavioral health system or transition to lower level of mental health services.
