Case Manager
About the role
We're hiring a Case Manager to help individuals and families achieve greater self-reliance. The ideal candidate will provide comprehensive, client-centered case management and ongoing support.
Responsibilities
- Provide comprehensive, client-centered case management and ongoing support.
- Assess clients' social and financial needs, establish goals, and develop individualized action plans.
- Connect clients with internal and community resources, including housing, employment, education, healthcare, mental health, childcare, transportation, and financial literacy services.
- Maintain accurate and timely case documentation in the agency's electronic data system.
- Monitor referrals, evaluate outcomes, and adjust service plans as needed.
- Build collaborative relationships with community partners and represent Utah Community Action professionally.
- Participate in outreach events, which may occasionally occur outside regular business hours.
- Ensure compliance with all agency policies and funding requirements.
Requirements
- Bachelor's degree in Social Work, Education, or a related field.
- Two years of case management experience.
- Proficiency with Microsoft Office.
- Valid driver's license and ability to travel as needed.
