Job Summary

The Masters of Social Work/Licensed Clinical Social Work (MSW/LCSW) Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager’s for guidance with medically complex cases. Handles complex cases requiring advanced social work knowledge.

Responsibilities

  • Collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans
  • Collaborate with RN Case Manager’s for guidance with medically complex cases
  • Handle complex cases requiring advanced social work knowledge
  • Facilitate coordination of care
  • Provide guidance and share expertise with other case managers, leads, and conduct training sessions on new resources and programs
  • Serve as a resource for discharge planning and care coordination for psychosocially complex cases
  • Maintain current knowledge of available community resources and post-acute care options

Qualifications

  • Graduate Degree required - Master's Degree in Social Work from school accredited by the Council on Social Work Education
  • 2 or more years of professional social work experience or 2 or more years of healthcare experience preferred
  • Case Management or related experience preferred
  • Licensed Clinical Social Worker (LCSW) Certification required

  • Knowledge/Competence of Microsoft Office products and keyboarding (preferred)

Physical Demands

The job requires frequent sitting, standing, walking, and bending/stooping. Frequent repetitive body movements. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull up to 20 lbs. occasionally.

Pay

The base compensation range for this role is $29.83 - $48.44 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.

Schedule

Weekend and holiday coverage required. May require on call responsibilities as directed and ability to travel between campus buildings, remote facilities, and out of town as needed.
