Case Manager III - Mental Health 135
About the role
Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Responsibilities
- Assist clients in accessing necessary community resources and services
- Develop and implement individualized service plans
- Provide crisis intervention and support
- Coordinate transportation for clients to appointments and groups
- Document client progress and maintain accurate records
- Participate in regular team meetings and case reviews
- Ensure compliance with state and federal regulations
Requirements
- Bachelor’s degree in Social Services or Human Services and (1) year of experience
- High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
Qualifications
- Willingness to use your personal vehicle to drive clients to appointments and groups
- Willingness to use the company vehicle to drive clients to appointments and groups
- Willingness to use your personal vehicle to attend meetings
Skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of mental health disorders and treatment options
- Proficiency in Microsoft Office Suite

Pay
Expected starting wage range is $22.17 - $27.08. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
Schedule
Shifts Available: Full-Time | DAYS | Shifts: 7:00 AM - 5:30 PM, 7:30 AM - 6:00 PM, 8:00 AM - 6:30 PM | Days vary as needed
