What You Will Do To Change Lives

To change lives under direct supervision, you will serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders. Your responsibilities include:

  • Assisting individuals to live as independently as possible in their chosen community.
  • Providing support and guidance to help clients manage their mental health conditions.
  • Collaborating with clients, families, and other healthcare professionals to develop and implement individualized treatment plans.
  • Maintaining accurate and up-to-date records of client progress and interactions.
  • Ensuring clients adhere to prescribed medication regimens and therapy sessions.
  • Facilitating access to community resources and services that enhance clients' quality of life.

What You Bring To The Table

  • Bachelor’s degree in Social Services or Human Services and (1) year of experience, or
  • High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services, or
  • An Associate's degree may be substituted for two (2) years of experience.
  • A valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • A willingness to use your personal vehicle to drive clients to appointments and groups, etc.

  • The pay range assigned to this role will be based on the geographic location from which the role is performed.
  • Starting pay is commensurate with relevant experience above the minimum requirements.
