About the role
Telecare’s AgeWise program provides community-based, strengths-based case management and institutional care coordination for older adults aged 59+. As a Case Manager III, you will support individuals with mental illness and co-occurring disorders to live as independently as possible in their community.
Responsibilities
- Partner closely with older adults to identify their needs and connect them directly to vital resources, including housing, medical care, psychiatric services, and natural support networks.
- Provide critical care coordination, advocacy, benefit navigation, and comprehensive discharge planning for conserved members placed in long-term care facilities, always striving to transition them to the least restrictive environment possible.
- Work within a highly collaborative team environment, providing direct support that empowers individuals to recover their health, hopes, and dreams.
Requirements
- Bachelor’s degree in Social Services or Human Services and (1) year of experience
- High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associate's degree may be substituted for two (2) years of experience
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
Qualifications
- Bachelor’s degree in Social Services or Human Services and (1) year of experience

Pay
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
Schedule
Full-Time | 8:00 AM - 5:00 PM | Monday - Friday
