About the role
The Case Manager I provides on-site shelter case management services to eligible clients. The role involves identifying and coordinating psychosocial, habilitative, and medical services to meet clients' needs.
Responsibilities
- Conducts casework interviews to assess clients' needs and formulate program/service eligibility.
- Identifies and assesses clients' psychosocial, economic, and physical needs, along with their support systems and available community resources.
- Develops and implements a case plan with intervention strategies to address clients' needs.
- Affords assistance with housing search and placement activities, and refers clients to appropriate community resources.
- Prepares and maintains accurate case notes, writes correspondence, reports, and other written materials, and inputs data into HMIS.
- Reviews assigned cases regularly to determine continuing need for services and participates in case consultations.
Requirements
- Bachelor’s degree in a social work-related field preferred.
- Two years of case work experience in a social service agency, or fieldwork; SOAR experience/training preferred.
Qualifications
- Education: Bachelor’s in a social work related field preferred.
