Case Manager for Jennings County - H&W/TBI Waiver

About the role

At IPMG, you will work in a hybrid model combining remote work from your home office with travel to community-based meetings within Indiana or within 10 miles of the state line, up to one hour per day.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Requirements

• A valid driver's license, car insurance, and reliable transportation
• RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Pay

The salary range for this position is not specified, but includes a sign-on bonus of $500 within the first 6 months.
