About the Role
This full-time, hybrid position combines remote work from your home office with travel of up to one hour for community-based meetings several times a week. To accommodate required in-person meetings, candidates must reside within Indiana or within 10 miles of the state line.

Responsibilities
- Provide information and education to help Individuals understand the Medicaid waiver process
- Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals served
- Help Individuals access services and supports they need to meet their goals
- Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
Who You’ll Work With
Care managers work with Individuals on the Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers.
Requirements
- Valid driver’s license, car insurance, and reliable transportation
- Designated home office setting with computer, internet service, and smartphone
- Ability to pass thorough background screens
Qualifications
- RN or
- Bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services or
- Any bachelor’s degree with 2 years full-time, direct service experience with the elderly or disabled or
- Master’s degree in social work, Psychology, Counseling, Gerontology, Nursing, or Health and Human Services
