Responsibilities

- Provide information and education to help Individuals understand the Medicaid waiver process. - Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve. - Help Individuals access services and supports they need to meet their goals. - Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Requirements

- A valid driver's license, car insurance, and reliable transportation. - RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services. - The ability to pass thorough background screens. - Designated home office setting with computer, internet service, and smart phone.
