Role Responsibilities
- Manage and maintain a caseload of individuals who are diagnosed with serious and persistent mental illness, substance use disorders and co-occurring medical conditions throughout the respective County.
- Visit clients at least once a month and twice annually in accordance with waiver-specific rules.
- Collapse and coordinate client intake and discharges, within set parameters and timelines.
- Collaborate with other professionals such as physicians, mental health providers, and other teams as needed to develop and update plans for each client.
- Ensure financial authorizations and provider contracts are in place.
- Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
- Access available community and housing resources so that necessary and relevant resources match an individual’s needs and preferences.
- Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community.
Required Qualification and Experience
- The Waiver Case Manager shall be qualified under Minnesota law as follows:
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology or a closely related field. OR
- Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
- Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
- Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.
Preferred Qualification and Experience
- Master's degree in behavioral science or related field.
- Experience providing direct services to adults with serious and persistent mental illness.
- Experience with case management and commitment proceedings.

Physical & Mental Requirements
- Employees in this role must be able to: Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques.
- Use hands for writing, typing, and data entry.
- Lift up to 40 lbs. and occasionally exert physical effort during duties.
- Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle.
