About the Role
The Registered Nurse (RN) Case Manager (CM) coordinates care and service for selected patient populations through the acute care episode, transitioning the patient across the continuum. The CM collaborates with interdisciplinary teams, both internal and external to Hendrick Medical Center (HMC), to improve patient care through effective utilization and monitoring of healthcare resources. The role assumes leadership to achieve desired clinical, financial, and resource outcomes.
Responsibilities
- Coordinate care for selected patient populations across the acute care episode and continuum.
- Collaborate with interdisciplinary teams to optimize healthcare resource utilization and monitoring.
- Lead efforts to achieve clinical, financial, and resource outcomes.
- Demonstrate advanced communication and interpersonal skills with medical staff, patients, families, clinical personnel, support staff, external agencies, and community members.
- Engage in complex clinical decision-making and interdisciplinary collaboration.
- Utilize strong critical thinking, analysis, and conflict resolution skills.
- Exhibit creative problem-solving and organizational planning abilities.
- Work proficiently with minimal supervision and tolerate frequent interruptions and demanding workloads.
Requirements
- Graduate of an accredited School of Nursing.
- 3 years of clinical experience as a staff nurse in an acute care hospital; Charge/Nurse Manager experience beneficial.
- Registered Nurse Certification through Commission for Case Manager (CCM) or American Case Management Association (ACMA) within 2 years of hire.
Skills
- Advanced communication and interpersonal skills.
- Strong computer skills, including proficiency in Allscripts EMR, Allscripts CM software, Milliman Care Guidelines (MCG), IDX, and 3M within 6 months.
- Critical thinking, analysis, and conflict resolution.
- Excellent human relations and creative problem-solving abilities.
- Organization, planning, and ability to work independently.
