About the role
A dynamic and collaborative team environment is at the heart of our organization. As a key member of our team, you will play a vital role in providing clinical case management services.
Responsibilities
- Assess and document patient needs, goals, and progress
- Develop and implement individualized care plans
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patient progress and adjust care plans as necessary
- Communicate regularly with patients and their families to provide support and guidance
- Participate in regular team meetings to share updates and collaborate on patient cases
Requirements
- Bachelor’s degree in a related field (e.g., nursing, social work)
- At least 2 years of relevant experience in clinical case management
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
Qualifications
- Current license to practice in the state of [State]
- Experience working with diverse populations
- Excellent organizational and time management skills
