About the role

RVNAhealth is seeking a compassionate and experienced Registered Nurse Case Manager to provide exceptional home health care throughout their service area of Danbury, CT and surrounding communities.

Responsibilities

  • Manage a caseload of home health patients, providing skilled nursing care and comprehensive case management.
  • Conduct Start of Care admissions, comprehensive assessments, and ongoing evaluations to identify patient and family needs.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary care team.
  • Carefully coordinate nursing, home health aide, therapy, and other support services to ensure high-quality, seamless patient care.
  • Maintain close monitoring of patient progress, communicate changes in condition, obtain physician orders as needed, and adjust care plans accordingly.
  • Educate patients and caregivers on diagnoses, medications, treatments, and self-management strategies to promote independence.
  • Complete timely, accurate clinical documentation in accordance with agency, state, and Medicare guidelines.
  • Participate in discharge planning and ensure smooth transitions when care goals have been achieved.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current Registered Nurse (RN) license in Connecticut or Compact RN License.
  • At least one year of nursing experience.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Home health experience and OASIS documentation experience are a plus but not required.

Pay

The target pay range for this position is $97,000 to $105,000 per year. Final salary will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
