Case Manager
About the role
As our Case Manager, you will engage the client in creating and implementing an individual service plan, identifying their self-identified goals and objectives, service needs, strengths, barriers, support systems, and emotional triggers. Using this plan, you will link the client to community services and work with them directly to provide support and training on living skills such as grocery shopping, budgeting, home upkeep, meal planning, social skills, coping strategies, and self-monitoring their psychiatric symptoms. Through regular home visits, you will monitor the client’s progress and behaviors and support the client’s goal attainment. This position includes client transportation as needed.
Responsibilities
- Create and implement individual service plans for clients
- Identify and link clients to community services
- Provide direct support and training to clients on living skills
- Monitor client progress and behaviors during regular home visits
- Support clients in achieving their identified goals
- Ensure client transportation as needed
Requirements
- Bachelor’s Degree in Social Work, Psychology, or related field
- Valid Georgia driver’s license with a clean driving record
- Satisfactory results of a criminal background check
Qualifications
- Experience working with individuals with mental health conditions
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and services
Skills
- Excellent organizational and time management skills
- Ability to build rapport and trust with clients
- Proficiency in Microsoft Office applications

Pay
$19-20.00/hour
Schedule
Full-time weekday community-based role: 8:30am – 5:00pm
