About the role
A Care Coordinator develops and implements personalized care plans, ensuring extraordinary resident care while managing costs and resources.
Responsibilities
- Communicate with residents and families about care plans and treatment protocols
- Participate in team meetings
- Monitor and adjust resident statuses
- Organize daily work and complete documentation
- Perform utilization reviews
- Coordinate admissions, discharges, and referrals
- Maintain positive relationships with residents, families, staff, and providers
Requirements
- Bachelor’s degree preferred
- Experience in healthcare, case management, or social services strongly valued
- Knowledge of Medicare, Medi-Cal, Medicaid, and electronic medical records is desirable
- Strong communication, organization, time management, and computer skills
- Compassionate, professional demeanor with excellent interpersonal skills
