About the role
Under the supervision of the Director of Case Management, you will participate as the patient program manager of the outpatient/inpatient team of professionals to meet discharge goals of patients and families, and monitor outcomes and utilization of services. As the RN/MSW Case Manager, you will use effective communication skills by communicating with payor representatives and providing age-appropriate clinical intervention to meet psycho-social and referral needs of identified patients and families.

Requirements
- Minimum of two years recent experience in a hospital or rehabilitation facility
- Minimum of two years recent insurance or case management experience preferred
- MSW or RN required
- CCM certification preferred
