About the role

The Case Manager position at Harbor is a full-time opportunity located at our Port Sylvania location. This role offers the chance to provide community-based support and resources to children and adolescents with various developmental and behavioral health needs. The successful candidate will work closely with an interdisciplinary team of therapists, psychiatric providers, and other professionals to deliver comprehensive, person-centered care.

Requirements

  • Minimum of a High School Diploma and 3 years of experience required.
  • Bachelor’s degree in Counseling, Social Work, Psychology, Nursing, or closely related behavioral/mental health field preferred.
  • Must be proficient and accurate in computer use, including Microsoft Word.
  • Must successfully complete required training before services are rendered, covering topics such as mental illness or substance use disorder treatment, crisis response procedures, and understanding the community behavioral health system.
  • Must successfully demonstrate the basic competencies listed above within 90 days of hire.
  • When applicable, non-licensed staff must complete 20 hours of CEUs every 24 months.
  • Must have reliable transportation, valid driver’s license, acceptable driving record, and be insurable through Harbor’s vehicle insurance carrier.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

Responsibilities

  • Accurately and timely document services using forms such as activity/progress notes, ISPs, DPS logs, incident report forms, and others.
  • Assess client needs and collaboratively develop a strength-based ISP with the client, family, or significant others.
  • Provide crisis prevention and intervention support, education, and consultation to clients, families, and/or significant others.
  • Maintain clients by identifying and minimizing the effects of psychiatric symptoms, and assist in building social support skills and networks.
  • Coordinate indicated evaluations and assessments by internal and/or external providers.
  • Monitor and coordinate all services outlined in the ISP.
  • Assist clients in accessing essential community resources.
