CASE MANAGER
Job Summary
Nestled in the Sierra foothills of California, Yosemite Treatment Center is a licensed Short-Term Residential Therapeutic Program offering individual, group, and family therapy, as well as evidence-based skill and rehabilitation groups. The Case Manager position is part of Rite of Passage's Integrated Care Model, which provides a trauma-responsive program tailored to each youth.
Pay and Benefits
Starting at $24.00 per hour. Perks & Benefits include medical insurance eligibility on the first of the month following 60 days of full-time employment, immediate access to the Rite of Passage Benefits Package after 90 days of full-time employment, and a 403b match of up to 6% after 1 year of employment. Additional benefits include paid time off that can be used as soon as it accrues.
Responsibilities
- Administer, develop, and facilitate the completion of case plans and treatment programs
- Develop and oversee the completion of the MAP Plan
- Provide monthly progress notes and weekly student updates
- Facilitate weekly group meetings
- Conduct assigned responsibilities with respect, consideration, encouragement, positive guidance, and utilization of resources towards the youths’ overall success
Requirements
- Be empathetic, patient, consistent, compassionate, have an honest work ethic, and treat others with respect
- Have a desire to work with troubled teens
- Have a BA/BS in a related field
- Have prior case management experience, including 3 years with youth and/or juvenile placement agencies
- Be at least 21 years of age
- Pass a criminal background check, drug screen, physical, and TB test
- Pass a search of the child abuse central registry
Schedule
5 days per week: Sunday - Thursday or Tuesday – Saturday, 8-hour shift with 1-hour break.
