About the role

Benchmark Human Services is launching a new program in Vermont designed to support individuals and families in planning, developing, selecting, accessing, coordinating, and overseeing medical, social, educational, and other services and supports. This includes providing planning, advocacy, monitoring, and guidance to help them make and evaluate their own decisions. We are hiring Case Managers to provide resources and support to individuals with intellectual and developmental disabilities, their families and guardians in order for the individual to be included in society, live as independently as possible and function at their maximum potential.

Essential Functions and Responsibilities

  • Must complete core competency training required by the State
  • Performs evaluations and assessments to meet the needs of individuals served
  • Affords and supports the individual to understand the goals and activities of the person-centered planning process, including assessment.
  • Facilitates and schedules participation in person-centered planning and assessment as needed.
  • Completes Needs Assessment Annual Review Protocol once a year with the individual and their team.
  • Affords the individual in discovery and exploration discussions, identifying preferences, strengths, goals and dreams, and needs in preparation for person-centered planning process.
  • Facilitates the completion of the DS Budget and Service Planning Context Document.
  • Affords the individual in choosing people to be part of the person-centered planning team who may contribute valuable information during the planning process.
  • Affords the individual to invite other people of the individual’s choosing (excluding legally prohibited individuals) to participate in person-centered planning meetings to occur at times and in locations selected by and convenient for the individual.
  • Works with the individual to develop agenda(s) and follow up notes for person-centered planning meetings and ensures this information is distributed to members of the support team as approved by the individual.
  • Supports the individual to lead, and provide assistance as needed, throughout the person-centered planning process.
  • Affords the individual and their support team in identifying any needed modifications to rights and freedoms
  • Reviews and updates person-centered service plan and related documents (including assessments), with the individual.
  • Maintains person centered service plan implementation, satisfaction, and health and well-being through visits with the individual and review of documentation, and through engagement with their guardian (if they have one), family members as applicable, and their service providers.
  • Reviews, assesses and responds to emergency situations and incidents and assures that actions taken are appropriate and timely to protect the health and welfare of individuals.
  • Validates that services provided are consistent with the ISA/ BIP person centered service plan, and the participant’s needs and desired outcomes.

Knowledge, Skills, and Abilities

  • Must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services
  • Fundamental understanding of state statutes, state rule, definitions, program requirements, program specific services, variances and exceptions.
  • Must be able to work collaboratively while meeting individual performance goals
  • Must be able to verbally and non - verbally communicate with a broad audience of stakeholders
  • Ability and skill to use technology tools in the daily performance of duties

Qualifications

  • Valid Driver's License
  • Valid Auto Insurance
  • Must live in the state of VT
  • Minimum of 1 year working in a similar position
  • Travel up to 75%
  • Knowledge of Vermont's public service system of developmental disabilities services and brain injury services
  • A bachelor’s degree in any field + 1 year of human services related experience such as supporting individuals with disabilities, special education, sociology, vocational rehabilitation and/or employment, protection and advocacy, psychology, substance use, aging, healthcare, social work, or other related field.
  • OR an associate degree in human services or a closely related field + 2 years of human services related experience
  • OR a high school diploma or equivalent + 3 years of human services related experience
