St Louis, MO, US, 63108-2403 Job Summary Catholic Charities of the Archdiocese of St. Louis is seeking a Case Manager to work within our Pathways to Progress Program. This role is responsible for providing comprehensive case management services to individuals and families experiencing social and economic challenges in Washington County, MO. Through assessment, service planning, coaching, resource coordination, and ongoing support, the Case Manager helps clients identify and achieve self-sufficiency goals. This position coordinates access to internal and community-based resources, maintains compliance with program and grant requirements, supervises case management activities and support staff as assigned, and promotes positive client outcomes through a strengths-based, empowerment-focused approach. This is a full-time position with full benefits and the annual compensation starts at $42,000. Job Responsibilities Conduct client intake, assessment, and reassessment activities to identify needs, strengths, barriers, and eligibility for services, and facilitate referrals to appropriate program and community resources.Develop, implement, monitor, and update individualized service plans in partnership with clients.Maintain an assigned caseload and provide services in accordance with established program service delivery models.Connect clients with appropriate internal and external resources and coordinate services with community partners.Monitor client progress and provide coaching, advocacy, and support to help clients achieve service plan goals.Maintain accurate, timely, and complete case records, reports, and documentation in accordance with ministry, program, and grant requirements.Respond appropriately to client crises by assessing needs, providing support, coordinating resources, and following established ministry protocols and emergency procedures.Serve as a liaison with community organizations and service providers to strengthen referral networks and client resources.Perform other duties as assigned. Job Requirements Bachelor's degree in Social Work, Human Services, Counseling, Psychology, Sociology, or a related field required; Master's degree preferred. Minimum of three (3) years of experience providing case management, social services, client advocacy, or related human services support. An equivalent combination of education and relevant experience may be considered.Experience conducting client assessments, developing service plans, coordinating community resources, and managing client outcomes.Valid driver's license (MO residents require Class E), personal vehicle, and proof of automobile insurance required.
