Responsibilities
- Maintain a caseload of 12-18 households (individuals and families) with chronic homelessness history
- Monitor and document client progress towards service plan goals
- Conduct regular case conferences with clients and service providers as needed
- Outreach clients, and conduct monthly home visits, and field work, with clients when needed
- Avoid maintaining familiarity with agency and city resources available to clients
- Maintain and direct all walk-in clients
- Document progress notes for all encounters and services provided to clients
- Participate in regular meetings, trainings and weekly supervision with supervisor and other support staff
- Complete ongoing Service Plans, Apartment Inspections, and Assessments for caseload
- Cook with clients, and manage and direct all walk-in clients
- Coordinate and connect clients to variety of medical, mental health and behavioral health physicians and services
- Assess ongoing eligibility for various entitlements and resources for caseload
- Meet monthly reporting requirements including timely documentation and engagement with full caseload
- Absolutely assist with client events, as needed
- Support program by assisting with client events, as needed
- Work one Saturday shift, from 8am-4pm, each month
- Perform additional duties as assigned
Qualifications
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment
- Bachelor’s degree in social work or another related field of study
- One year of case management or relevant work experience preferred
- Engagement and detailed oriented skills are essential
- Proficiency in Microsoft Office suite and other standard business technology is required
- Strong oral and written communication, time management and organizational skills are necessary
Physical Requirements
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
