Responsibilities

  • Independently manage a caseload.
  • Conduct comprehensive intake assessments.
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals.
  • Work with clients to develop goals and service plans.
  • Provide referrals and direct assistance to support clients applying and/or attending appointments for mental health services, medical care, substance treatment, vital documents, benefits, Legal/immigration services, and permanent housing.
  • Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals.
  • Maintain familiarity with social services and resources available both on-site and in the community.
  • Cookordination client care with other service providers; attend regular case conferences with clients and other service providers, as required.
  • Attend regular supervision sessions with direct supervisor.
  • Drive agency vehicle to and from client locations and appointments.
  • Perform additional duties as assigned.

Qualifications

  • Bachelor’s degree in social work or a related field of study.
  • Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points.
  • One-year of case management or related work experience preferred.
  • Ability to engage and build rapport with populations with high support needs.
  • Strong oral and written communication skills.
  • Detail-oriented and resourceful with strong time management and organizational skills.
  • Proficiency in Microsoft Office suite and other standard business technology.
  • Bilingual/multilingual preferred but not required.

Physical Requirements

  • Excellent map reading skills and ability to navigate public transportation system.
  • Excellent organizational and interpersonal skills.
  • Strong grammar skills.
  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to climb five flights of stairs.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather.
